AngelNet Sign-Up Form	AngelNet is a network of volunteers who are willing to help the needs of fellow parishoners.  See Ministries & Outreach
Annual Pledge Card	The Annual Pledge Card is your pledge to support St. Barnabas
Church School Registration Form	Please fill out the registration form for any children attending Church School.  All children are required to register each year in August/Septemeber.
Membership Transfer Form	Fill in this form if you want to transfer your membership from your current Episcopal church to St. Barnabas
Family Information Form	Please fill in this form to provide St. Barnabas with your family information (the purpose of this information is to better serve you as new members of St. Barnabas)
Pledge Payment	Please designate your contribution as part of your annual pledge
Payment/Reimbursement Form	If you have made any expenses on behalf of St. Barnabas, you are entitled to a reimbursement.
Permission Form	Medical and/or transportation permission form for youth programs sponsored by St. Barnabas
VBS Registration Form	Sign up your child (PreSchool - 5th Grade) for the VBS program in the Summer.  Check the VBS page for more information.
Youth Volunteer Hrs	If you have volunteered time to St. Barnabas, and need credit, please fill out this form and have it signed by the Chair of Jr./Sr. High
30th Anniversary Survey	Help us learn more about you, fill out the survey the information you provide will be kept confidential.
